If you are a salaried worker, do not pay Employment Insurance premiums
but pay premiums under the Québec Parental Insurance Plan, your
employer must fill out the Relevé de renseignements sur l’emploi –
Régime québécois d’assurance parentale form.
Once your employer has completed the form, you must submit it. You can either
upload it to your online file or send it by mail to the following address:
